Responsibilities

What You\u2019ll Do

• Deliver nursing care as outlined in each patient\u2019s personalized care plan


• Partner with RNs, therapists, and other clinicians to ensure top-quality care


• Monitor and report patient conditions and changes with accuracy


• Build meaningful relationships with patients and their families


• Provide comfort, dignity, and hope with every visit

Qualifications

What You Bring

• Active LPN license in Pennsylvania


• A passion for home health and patient-centered care


• Strong communication and time management skills


• The ability to work independently\u2014with full team support behind you
