Job Description

Wellspring Community is seeking a Part Time Direct Support Provider to join our Residential Team in Castle Rock. A Direct Support Professional is more than just helping someone with their daily activities; it is assisting someone to live a meaningful life. Direct care support is a valuable career that changes the lives of the individuals you support and your life, too! In this role, you will be a mentor, friend, and advocate to the individuals you support.   


 


Part Time Schedule: Varies Two afternoons/evenings and every Saturday; 20 -24 hours weekly 


 


Essential Responsibilities and Job Duties:
 



  • Provide direct care and supervision for people with intellectual and developmental disabilities in the community and in their personal home.  



  • Assist individuals in completing living skills, money management, cooking, and community participation, hygiene routine, household cleaning, and grocery shopping   



  • Plan and carry out a variety of recreational activities to advance social development (outings to museums, parks, dances, recreation center, etc.).  



  • Provide transportation to individuals as needed.  



  • Assist with medical care (medication administration, and transportation to medical appointments).  



  • Implement and document individual goals and medical tracking electronically.  



  • Participate with the team in development and implementation of the Individual Service and Support Plan.  



  • Coordinate other resources as needed, including support in arranging and/or directly providing transportation for participants.  



  • Facilitate and monitor activities consistent with Individual Service Plan goals and objectives for assigned caseload.
     



  • Responsible for supporting and maintaining a physically and psychologically safe environment.  



 


Expectations of Position: 
 



  • Able to work well with team members in all departments within the Wellspring Community and the public at large.   



  • Well-developed communication skills, both written and verbal.  



  • Demonstrates ability to manage challenging behaviors through the execution of positive behavior supports for each individual/unique situation or seek help from senior positions when necessary.

     



  • Excellent time management skills  



 



Qualifications & Skills: 
 



  • High School Diploma or Equivalent required.  



  • Experience working with the IDD community or other at-risk population   



  • Experience working with Electronic Health Records (EHR)   



  • Valid Driver’s license  



  • Proficiency in computer skills including Microsoft Office and Electronic Charting Systems.  



  • Must be able to pass a background check, drug screen, and obtain DOT certification.
