Full Time Home Health Physical Therapist (PT)

Overview

We are seeking a Full-Time Physical Therapist (PT) for a very flexible home health position serving adult and geriatric patients. This role offers independence, strong support, and good pay. 

What You\u2019ll Do:

• Evaluate patients referred by physicians using diagnosis-specific assessments




• Provide in-home PT visits for adult and geriatric patients




• Complete timely documentation (evaluations, progress notes, time & mileage)




• Schedule and manage your daily patient visits




• Collaborate with physicians and interdisciplinary team members




• Communicate patient progress and updates as needed

Requirements:

• Bachelor\u2019s, Master\u2019s, or Doctorate in Physical Therapy




• Active PT license in the state




• Must be a U.S. Citizen or Green Card holder (no exceptions)




• Comfortable with driving between patient homes (Most patients are close together; occasional 20\u201340 min drive. Mileage reimbursed.)
