Responsibilities

• Perform physical therapy admissions, evaluations and reassessments


• Develop individualized treatment plans


• Provide skilled physical therapy services in the home setting


• Supervise Physical Therapist Assistants (PTAs)


• Improve patient mobility, strength, balance, safety, and independence


• Educate patients and caregivers


• Collaborate with physicians and interdisciplinary team members


• Complete documentation accurately and timely

Qualifications

• Active California Physical Therapist License


• Valid driver's license


• Strong communication and documentation skills

Home health experience preferred, but not required.
