Overview


This role as Direct Care Staff will provide client-centered and client-driven services in client homes (group setting) that will focus on community support in apartment, conflict resolution, symptom management, group services/outings, and more.

We have a variety of part-time shifts available at our Madison, IL housing site:



  • Every other Saturday day shift

  • Every other Sunday day shift

  • Every Saturday day shift

  • Every Sunday day shift

  • Every Sunday evening shift

  • Every Monday evening shift

  • Every Saturday and Sunday


Responsibilities



  • Meeting Clients Where They Are: You won't be stuck behind a desk. You\u2019ll provide support in the environments where clients feel most comfortable\u2014whether that\u2019s in their own homes or out in the community.


  • Building Custom Care Plans: Using the IM-CANS assessment tool, you\u2019ll help clients design a personalized roadmap for their recovery based on their specific goals and needs.


  • Providing Hands-On Support: You will act as a guide and advocate, offering case management, teaching daily living skills, and helping clients manage and understand their medications.


  • Leading Therapy & Skill-Building: You\u2019ll facilitate individual, family, and group therapy sessions using proven, science-backed methods (Evidence-Based Practices) to help clients build a more independent life.


  • Managing the Essentials: You will handle the "behind-the-scenes" details, such as pre-authorizing and re-authorizing services to ensure care remains uninterrupted.

Qualifications



  • Must be at least 21 years of age and have a high school diploma or equivalent plus 6 months\u2019 peer or social service experience.

  • Demonstrated skills in human services and in services to adults.

  • Demonstrated ability to work within organizational structure; to accept supervision; and to work constructively with consumers, treatment resources, and the community.

  • Basic computer skills including MS Word and an electronic medical record. Valid driver\u2019s license, private auto insurance, and be insurable.

The anticipated starting pay for new hires for this position is between $21.35 - $24.31 an hour. There are several factors taken into consideration in determining base pay, including but not limited to: job-related qualifications, skills, education, experience, local market conditions, and internal equity. Check out additional benefits here!
